Assistant Director of Nursing (ADON)

The Assistant Director of Nursing is the senior clinical and supervisory authority in the building during the overnight shift and will report directly to the Director of Nursing (DON). The ADON is responsible for the safety and quality of resident care between 11:00 pm and 7:00 am, Monday through Friday, across all three units, and for the direction, oversight, and accountability of every nursing and direct-care employee working those shifts.This is a working supervisory position, not an administrative one. The ADON is expected to be physically present on the units, rounding, observing care, and available as an immediate clinical resource. The position also assumes defined regulatory, training, and audit responsibilities that support the Director of Nursing. Essential Functions Supervision and staff accountability Provide direct, on-site supervision of all nursing and direct-care staff assigned to the overnight shift across all three units. Assign, direct, and adjust work throughout the shift; verify that assigned care tasks, rounds, and unit duties are completed. Conduct and document a minimum of two supervisory rounds per shift on each unit. Address performance and conduct concerns in real time, including cell phone use, sleeping on duty, failure to remain in resident areas, and failure to follow direction; document observations factually and route to the Director of Nursing and Human Resources. Manage overnight call outs and coverage decisions, including reassignment of staff across units to maintain required coverage. Serve as preceptor and competency validator for nursing and direct-care staff orienting on the overnight shift. Clinical oversight Act as the first clinical resource for LPNs and aides on change in condition, falls, behavioral events, and emergencies. Assess residents directly when a change in condition is reported; determine the appropriate response and initiate physician, family, and emergency service contact per policy. Oversee medication administration practice on the overnight shift, including observation of technique, documentation accuracy, and adherence to physician orders. Complete scheduled medication cart audits and controlled substance reconciliation across all three units on a defined cycle. Review overnight documentation for completeness and accuracy before the end of shift. Support residents living with dementia and their caregivers using person-centered, non-pharmacological approaches consistent with facility standards. Regulatory and compliance Recognize reportable events occurring overnight, initiate the incident report, and begin the 24-hour notification clock; prepare first-line documentation for review by the Director of Nursing and authorization by the Administrator. Ensure overnight staffing and care delivery meet the requirements of 18 NYCRR Part 487 and 10 NYCRR Part 1001, including 24-hour awake coverage and supervision of enhanced and special needs residents. Maintain survey readiness on the overnight shift, including documentation, medication records, and staffing records. Protect resident health information consistent with HIPAA and facility policy. Follow and enforce infection control, standard precautions, and OSHA blood borne pathogen requirements. Training and development Deliver and document required in-service education for overnight and off-shift staff, who are the most difficult population to reach through day-shift training. Identify skill gaps observed during rounds and convert them into targeted training. Maintain accurate in-service records and route completed documentation to Human Resources. Communication and administration Provide a written shift report to the Director of Nursing before 7:00 am. each shift worked. Participate in nursing leadership meetings, quality assurance activities, and the facility falls committee as scheduled. Communicate professionally and respectfully with residents, families, physicians, and staff at all times. Perform other duties consistent with the scope of the position as assigned by the Director of Nursing. Preferred Qualifications Bachelor of Science in Nursing. (required) Current, valid, and unrestricted license to practice as a Registered Professional Nurse in New York State.(required) Working knowledge of New York State regulations applicable to adult care and assisted living facilities. Prior experience as an ADON, Unit Manager, Nursing Supervisor, or Director of Nursing. Experience in a memory care, enhanced assisted living, or special needs assisted living setting. Experience preparing or submitting incident reports through HCS or HERDS. Experience delivering staff in-service education. Satisfactory background check, pre-employment health assessment, and tuberculosis screening as required by facility policy and applicable regulation. Physical Requirements and Working Conditions Remains mobile and on foot for the majority of an eight-hour shift, moving between three units. Able to respond quickly to an emergency anywhere in the building. Assists with resident transfers and repositioning; able to lift, push, or pull with assistance as required by facility safe-handling policy. Bends, stoops, reaches, and kneels in the course of providing and observing resident care. Sustained work on a permanent overnight schedule. Exposure to blood, body fluids, infectious materials, and cleaning agents, with personal protective equipment provided. Interacts with residents who may exhibit confusion, agitation, or resistance to care.
