MDS Coordinator

Location Lake Wales, FL AND/OR Lakeland, FL AND/OR Starke, FL. Role Description We have three different companies/Skilled Nursing facilities hiring for full time MDS coordinators in each building. The MDS Coordinator is a full-time, on-site role based in Lake Wales, FL, responsible for overseeing and managing the Minimum Data Set (MDS) assessment process in a long-term care or skilled nursing environment. Day-to-day responsibilities include coordinating and completing resident assessments, ensuring accuracy and timeliness of MDS documentation, and supporting interdisciplinary care planning. The role involves collaborating with nursing staff and other clinical team members to develop, update, and implement individualized care plans aligned with regulatory requirements and facility policies. The MDS Coordinator monitors changes in resident status, helps optimize reimbursement through accurate coding, and participates in quality improvement initiatives related to resident outcomes and compliance. This position requires consistent communication with leadership and clinical teams to maintain comprehensive, person-centered care. Qualifications Candidate should possess strong clinical credentials such as Licensed Practical Nurse (LPN) OR Registered Nurse (RN) experience and knowledge of geriatric nursing principles. Candidate should possess expertise in MDS coordination, including completing and reviewing MDS assessments and understanding related regulatory guidelines.
