RN Case Manager - Mental Health (44603)

Job Details Level: Experienced Job Location: Dilkon Medical Center - Dilkon, AZ 86047 Position Type: Full-Time Education Level: Associate Degree Travel Percentage: Occasionally Job Shift: Day Responsibilities Work under general supervision of the Chief of Mental Health (MH)/Psychiatrist, independently coordinating care for pediatric, adolescent, adult, and geriatric patients with mental and emotional disorders at Dilkon Medical Center and other Winslow Indian Health Care Center sites as needed. Provide support, counseling, assessment, crisis intervention, referral, and coordination of services for psychiatric patients in accordance with WIHCC/DMC, Indian Health Service, and national accreditation standards. Provide services primarily at the Dilkon Medical Center Mental Health Department; also work remotely, on-site with MH staff in Winslow and Leupp clinics, and interview WIHCC beneficiaries in outside facilities to facilitate crisis care. Assist in crisis cases by working with licensed medical providers to facilitate interventions, placement, and follow-up arrangements. Offer support of direct care to MH patients under protocols approved by MH Director and Chief Medical Officer, including psychotropic medications and injections when ordered by licensed providers. Collaborate with the Navajo Nation Division of Social Welfare and other agencies to coordinate services for patients reported for suspected child abuse/neglect; maintain liaisons with courts, social services, and medical community. Conduct home visits when necessary for supportive case management services to individuals and families. Work with patients in clinical settings or at homes to maximize benefits of treatment plans; implement strategies to enhance adherence to appointments, medications, and treatment recommendations; assist patients with transportation, interpersonal, financial, and legal issues as needed. Work actively with the Navajo Nation Regional Behavioral Health Authority (RBHA) and WIHCC Patient Benefits Coordinators to assure eligibility determination and enrollment in benefits programs. Collaborate with and refer to appropriate departments and clinical staff at DMC and WIHCC, including Medical Clinics, Emergency Department, Urgent Care, Maternal Child Health, RN Care Coordinators, Pediatric Care Coordinator, Public Health Nursing, Nutrition Service, WIC, Traditional Healing Program, Massage Therapy, Little Colorado Medical Center, and outside specialty consultants. Develop and maintain internal patient care database for continuous tracking and follow-up of active patients. Integrate quality, safety, efficiency, and cost‑effectiveness factors into planning, delivery, monitoring, and evaluation of patient care. Maintain compliance with applicable laws regarding patient confidentiality, abuse reporting, consent principles, and advanced medical directives. Advocate for patients and families at the service‑delivery level and community level, fostering decision‑making, independence, and development. Utilize best‑practice models to identify, develop, and implement mental‑health case‑management practices. Coordinate and track referrals with WIHCC providers and outside specialists; collaborate closely with inpatient and outpatient RN Case Managers. Function as a liaison for patients and providers by monitoring scheduled clinic appointments, referrals, travel accommodations, and specialized testing (CTs, MRIs, etc.). Communicate with courts and law enforcement to assure patients receive necessary treatment and that the mental‑health system works with the legal system to achieve optimal outcomes. Uphold all principles of confidentiality and patient care to the fullest extent. Assist when needed in counseling patients and crisis intervention, referring to appropriate providers. Perform chart reviews for high‑risk patients; identify needs, monitor adherence to treatment plans, and develop effective interventions. Establish and maintain an accurate registry for patients with suicide attempts or recent hospital admissions; actively track and follow up on patients not seen per standards of care. Develop case‑management guidelines and policies relevant to mental‑health issues. Provide individual and group education to relevant staff on mental‑health issues. Interact in an honest, trustworthy, and dependable manner with patients, employees, and vendors. Demonstrate cultural awareness and sensitivity. Maintain regular attendance and punctuality. Perform other duties as assigned by the supervisor. Qualifications Completion of an accredited nursing program and an unrestricted license as a Registered Nurse. Advanced experience as a Registered Nurse required. Minimum of one year of clinical experience in general outpatient healthcare services; experience in the mental‑health field preferred. Current BLS/CPR certification required. Bilingual skills in English and Navajo language preferred. Valid unrestricted and insurable driver’s license. Successful completion of a background investigation and compliance with Child Care position requirements for the Indian Child Protection and Family Violence Prevention Act. Knowledge, Skills, and Ability Dependable attendance and job performance. Ability to meet attendance and reliability requirements, including working additional hours when needed. Knowledge of basic human development, mental and emotional disorders, and substance use disorders. Knowledge of patient care service scopes and referral processes. Knowledge of effective teaching processes and health‑care planning and evaluation. Knowledge of quality management and improvement processes, including accreditation standards. Knowledge of clinic practices and ability to serve as a resource for clinic personnel. Knowledge of EHR, RPMS, iCare, and other WIHCC software; proficiency in Microsoft Office and Outlook. Knowledge of documentation standards and ability to document clinical data accurately. Knowledge of cultural and language differences and sensitivity. Knowledge of interpersonal skills for interacting with staff, agencies, groups, patients, and families. Ability to work as a team member and develop productive relationships with healthcare providers internally and externally, including judicial and law enforcement agencies. Physical Demands Regularly walk, sit, use hands to handle and manipulate objects and equipment, reach with hands and arms, and communicate effectively verbally and audibly. Frequently stand, climb, balance, stoop, kneel, crouch, or crawl; occasionally use taste or smell; occasionally lift and/or move objects weighing up to 50 pounds. Work Environment Performed in an office setting or outdoor work environment with moderate noise levels. Exposure to physical risks such as blood‑borne pathogens, hazardous chemicals, or operating potentially dangerous equipment. Requires adherence to all safety protocols. Scheduling may include evening, weekend, overnight shifts, extended hours, irregular schedules, and rotation as operational needs dictate.
