Nurse Admission/Discharge Coordinator
The Nurse Admission/Discharge Coordinator Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care. The nurse is an active member of an interdisciplinary team whose primary focus is on case management and care transitions to and from an inpatient setting. Job duties include, but are not limited to:
- Assist with patient planning assessments, identification of barriers, and activities required to successfully transition the patient in and out of the acute inpatient setting
- Apply the nursing process which includes systematic data gathering, assessment, and appropriate nursing judgment to ensure positive patient outcomes
- Coordinate and collaborate with other VA and non-VA departments, community agencies, and healthcare providers to ensure patient needs are addressed
- Collaborate in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines
- Demonstrate performance and leadership that is broad enough to improve the care for a group of patients
- Provide peers with formal or informal constructive feedback for improvement
- Support colleagues and other nurses through knowledge sharing to provide safe, quality nursing care
- Evaluate outcomes of evidence-based decisions and practice changes for individuals, groups, and populations
- Responsible for the documented outcomes at the program or service level

Pay Schedule: NG57
Pay: Competitive salary, regular salary increases, potential for performance awards
Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA

Licensure: 1 full and unrestricted license from any US State or territory
