Clinic Coordinator/Triage Nurse (RN)

Description Position Overview Under the supervision of the Clinic Director in a Rural Health Clinic (RHC) setting, this exempt employee coordinates day-to-day operations and clinic staff; triages patients calling into or presenting at the RHC as requested. Duties and Responsibilities Develops staff work schedules for clinical and non-clinical RHC staff. Maintains provider’s work schedule including call schedule. Coordinates urgent care coverage which may include collaborating with contracted healthcare provider agencies and individuals. Monitors and tracks staff attendance and punctuality according to policy. Assists Rural Health Clinic Director in addressing any staff call-ins and finding suitable replacements. May act as a fill in when necessary (Must be able to perform duties of Clinic Registered Nurse and Patient Access Clerk). Reviews staff timecards and prepares payroll system to ensure accurate staff payment. Assists Rural Health Clinic Director with new hire interviews. Participates in monthly staff and provider meetings and other relevant meetings as needed. Triages patients via telephone or in-person to determine the urgency of their need for treatment and the nature of treatment required. Responds to triage requests in a timely, professional manner. Assesses patient condition/symptoms and gives appropriate clinical direction, education, and recommendation(s) for disposition. Assesses needs using standard evidenced based protocols for triage, offers clinical recommendations, as well as referrals to health care providers, services and community resources. Documents accurately and timely in patients’ electronic health record. Gathers and submits Healthcare Effectiveness Data and Information Set (HEDIS) performance improvement data. Effectively communicates with patients, families, providers, outside agencies, and hospital personnel. Treats patients with respect and dignity at all times. Provides education for patient and family. Communicates in a way that patient and family can understand. Reinforces education as needed. Effectively communicates HEDIS measure specifications to Clinic Director. May assist Director with using HEDIS measure specifications for internal quality improvement activities. Maintains confidentiality according to policies and procedures. Assists in inventory and ordering of supplies and equipment. Assists in maintenance of safe, clean work environment. Assists with the general operation of the RHC. Assumes other duties and responsibilities that are related and appropriate to the position and area. Minimum Education and/or Experience Required Minimum of Associate Degree in Nursing (ADN). Bachelor of Science in Nursing (BSN) preferred. Previous experience in a medical office preferred. Additional Qualifications Must have strong computer skills including navigating online websites and working knowledge of Microsoft office products. Ability to learn and navigate Electronic Health Record. Ability to interact in a positive manner and to communicate well with patients, families, colleagues, director, physicians, and other medical center employees. Ability to use discretion in dealing with the public. Above average critical thinking and analytical skills required. Versatile and willing to coordinate with and participate in, when necessary, all other aspects of the Rural Health Clinic within scope of practice. Ability to handle confidential information. Must have patience, tact, and cheerful disposition and enthusiasm, as well as willingness to handle difficult situations. Demonstrated excellent interpersonal skills (listening, non-verbal communication, tone, empathy). Must be able to interact with fellow employees in a way that promotes a harmonious and cooperative working environment. Excellent oral and written communication skills required. Self-motivated. Licensure/Certification Required Current licensure as a Registered Nurse (RN) in the State of Iowa. Basic Life Support (BLS). Mandatory Reporter.
