CNA - LTC/SNF

Job Description

Job Summary:

Provides direct patient care support in hospital, clinic, or long-term care settings, assisting with daily activities, personal hygiene, and basic clinical tasks under supervision. Ensures patient comfort and safety across various care units and formats, including home care as needed.

Responsibilities:

• Assist patients with bathing, grooming, dressing, and personal hygiene activities.
• Support patients with toileting needs, including bedpans, commodes, and bladder/bowel training.
• Measure and record vital signs, intake/output, and other clinical observations accurately.
• Collect specimens and prepare patients for procedures, adhering to safety and infection control policies.
• Position, transfer, and mobilize patients to promote safety and comfort.
• Transport patients within care facilities and participate in recreational and therapeutic activities.

Qualifications:

• Certified Nursing Assistant (CNA) certification required.
• Proven experience in patient care settings, including LTC or SNF preferred.
• Excellent communication and interpersonal skills.
• Ability to follow care plans and implement routine treatment procedures.
• Knowledge of infection control, safety policies, and patient confidentiality.
• Capable of handling multiple patients and prioritizing tasks in a fast-paced environment.
