CPhT preferredThe Medication Therapy Management (MTM) Coordinator partners with our Medication Therapy Management (MTM) pharmacists to ensure the department runs smoothly and can care for as many patients as possible. Find a fulfilling career and make a difference with UnityPoint Health.ResponsibilitiesPatient Outreach, Scheduling, and Follow-upProcess referrals to the MTM team in utilizing Epic and schedule as neededResponsible for identifying care gaps for patients, proactively outreach patients to schedule appointments on behalf of the MTM pharmacists and teamGreet patients and check them in for face-to-face appointments as neededProvide phone support, including accurate messaging and spelling, screening, scheduling appointments, and directing callers to appropriate team members or clinics as neededProvide appropriate and timely documentation in the patient’s electronic health record using standardized workflows and processesFollow-up with patients between visits to collect, document, and communicate accurate information including medication list changes, blood sugars, and other patient-specific informationBack Office SupportUnder supervision of the pharmacist, help complete paperwork such as prior authorizations or copay assistance programs to help patients access affordable medicationsReview charts to ensure MTM billing requirements are metCollaborate with payors to ensure good communication on share goals such as HEDIS metricsDepartment SupportTriage patient lists generated from payors or UPH stakeholders to assign to the appropriate pharmacistAssist with data collection for internal projectsQualificationsEducation: High School degreeExperience:At least one (1) year of Healthcare-related experienceLicense(s) /Certification(s):Valid driver’s license when driving for any vehicle for work-related reasons.CMA or CPhT preferredKnowledge/Skills/Abilities:Strong interpersonal skillsAbility to PrioritizeStrong professionalismPatient-focusedBasic understanding of Microsoft ExcelJob SummaryRequisition ID: 2026-183055Category: PharmacyLocation: US-IA-West Des MoinesAddress: 1776 West Lakes PkwyAffiliate: 9010 System Services AdministrationCity: West Des MoinesDepartment: Medication Therapy ManagementState: IAFTE: 1.0FLSA: Non-ExemptScheduled Hours/Shift: 8Work Remotely within the US: YesWork Type (Portal Searching): Full Time Benefits
