Care Coordination Manager

Plan, implement, and administer comprehensive service delivery in case management, progression of care, social work services, discharge planning, and transitions in care.

Promote/establish effective communication between Care Coordination and other Hospital departments and members of the medical staff.

Keep informed regarding current regulatory and professional trends, changes, and standards applicable to the scope of Care Coordination and make appropriate recommendations or changes in policies, procedures, and/or programs to conform to regulation changes and standards.

Keep staff informed of Hospital Organizational goals, strategic plans and initiatives, finances, and Senior Management communication so as to ensure identification with the overall Hospital mission and goals.

With Managers, establish effective lines of communication for the completion of objectives, oversight of areas of responsibility and to ensure consistency for all Department staff.

Oversee the continuous assessment and improvement of the care coordination department.

Report pertinent information regarding patient problems, personnel problems, progress re: projects and initiatives and any unusual occurrences to the Vice President.
