POSITION SUMMARYThe Care Center Registered Nurse (RN) is a member of the interdisciplinary team who provides hospice/skilled professional nursing care as prescribed by the physician and in compliance with the medical plan of care. The Care Center RN is committed to the philosophy, goals, and objectives of Angela Hospice and the Felician Franciscan Core Values.POSITION REPORTS TO: DIRECTOR OF INPATIENT SERVICESESSENTIAL JOB FUNCTIONSCompletes an ongoing comprehensive assessment of patient and family to determine hospice needsProvides quality, professional nursing care to patients utilizing specialized nursing skills related to end-of-life careMay initiate the patient’s plan of care and includes the patient and the family in the planning processPerforms comprehensive assessment of assigned patients in a timely manner upon admission and as requiredAssesses and evaluates patients and makes necessary revisions to the plan of care as patient status and needs changeInitiates appropriate preventive and rehabilitative nursing proceduresAdministers and documents medications and treatments as prescribed by the physicianOrders medications as needed from pharmacyEducates the patients and families regarding medications, treatments and procedures, disease process, and end of life care/processUses established confidential communication channels to communicate pertinent informationCommunicates patient/family concerns to appropriate interdisciplinary team member and initiates a corrective action if requiredAssists the patient with activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort careLifts, moves, and transports patients using proper body mechanics or lifting devicesCompletes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care givenRecords pain/symptom management changes/outcomes as appropriateCommunicates with the physician and/or nurse practitioner regarding the patient’s needs and reports changes in the patient’s conditionCommunicates and collaborates with community health related persons to coordinate the care planTeaches the patient and family/caregiver self-care techniques as appropriateProvides and maintains a safe environment for the patientWorks in cooperation with the family/caregiver and hospice interdisciplinary group to meet the physical, emotional, and spiritual needs of the patient and family/caregiverAttends interdisciplinary group meetings as neededEnsures that arrangements for equipment and other necessary items and servicesare availableMaintains the comfort, privacy, and dignity of each patient and interacts with patients in a manner of compassion and empathyReports an incident or evidence of abuse or violation of residents rights to the Lead Nurse, Director of Inpatient Services, or any administrative staffProvides clinical supervision of LPNs, SNAs, hospice aides, and volunteers and delegates responsibilities when requiredThe above statements are only meant to be a representative summary of the major duties and responsibilities performed by the occupant of this job. The Care Center RN may be requested to perform job related tasks other than those stated in this description.OTHER DUTIESMaintains knowledge of and compliance with current Medicare/Medicaid, state/federal rules and regulations for hospice servicesEnsures compliance with the Medicare conditions of participation and other state regulations that govern the provision of healthcareComplies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state and organizational policiesParticipates in organizational monitoring of the quality of medical services and quality improvement initiativesDevelops, maintains and upgrades professional knowledge and practice skills through attendance at seminars, conferences and participation in continuing education and in-service classesFulfills the obligation of requested and/or accepted assignmentsDemonstrates knowledge in counseling patient/family in dealing with end-of-life issuesSupports department operations as directed as well as being assigned to any special projectsPerforms tasks which are supportive in nature to the essential functions of the position, but which may be altered based upon the need of the organizationMeets on-going requirements to remain compliant with Federal/State and/or other regulatory guidelinesParticipates in efforts and initiatives to promote growth and longevity of the organization within your departmental scopePerforms other duties as directed by the President/CEO, Director of Inpatient Services, or other assigned designeeQUALIFICATIONSGraduate of an accredited school of nursingCurrent license in the State of MichiganCPR CertifiedHospice experience in an inpatient or home care setting preferredExcellent organization and time management skillsProficient computer knowledge and skillExcellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklistAbility to work with little or no supervisionCapacity to lift, position and/or transfer patientsAuditory and visual skillsProficient interpersonal relations and communicative skillsKnowledge of Federal/State, NHPCO, and CHAP regulations and guidelinesSTANDARD REQUIREMENTSSupports the Felician Franciscan Core Values, and Vision of Angela Hospice:Respect for Human DignityProvides an individualized/person centered approach in all interactionsRespects and honors differing cultures, viewpoints and beliefsCompassionDemonstrates patience and kindnessGoes the extra mile to meet others’ needsTransformationRepresents the organization positively in the workplace and in the community as demonstrated through appearance, words and actionsTeam player – supports and encourages others; works with other team members to achieve goalsAccountability/ownership – knows and understands responsibilities of job; adheres to organizational policiesSolidarity with People in NeedProvides love, care and support for all; regardless of financial circumstanceSupports those in the organization/community needing helpJustice and PeaceTreats the organization’s and patients’ property with care and respectResolves conflicts in a positive, constructive mannerSUPERVISORY RESPONSIBILITYThis position does require the ability to supervise LPNs, SNAs, hospice aides, and is also required to orient employees as needed and will act as a mentor to staff and volunteers.PHYSICAL REQUIREMENTSThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. The position requires sitting and standing for long periods. The position is also exposed to infectious diseases and requires lifting of a minimum of 50+ pounds regularly. It also requires bending, stooping, turning and twisting on a regular basis. He/she requires the ability to operate a computer, phone and other office equipment as needed. Manual dexterity is also required to perform tasks including computer tasks.WORK ENVIRONMENTThis position operates in a health care setting. The role requires walking to various areas of the building and working directly with patients and other team members.EXPECTED HOURS OF WORKThis is a position in a hospice care facility open 24 hours a day, 365 days a year.
