Compensation:

$47,500 - $55,000 yearly

Responsibilities:

• Provide compassionate support and guidance to families navigating the complexities of aging and legal challenges.
• Collaborate with clients to develop personalized care plans that address long-term care, nursing home Medicaid, and estate planning needs.
• Act as a liaison between clients and community resources, ensuring access to necessary services and support.
• Offer expert advice on navigating emotional and legal challenges, empowering clients with knowledge and confidence.
• Maintain accurate and confidential client records, documenting interactions and progress to ensure seamless service delivery.
• Participate in team meetings and case reviews, contributing insights and strategies to enhance client outcomes.
• Stay informed on industry trends and legal updates, continuously improving your expertise and service offerings.

Qualifications:

• Experience in social work or a related field, with a focus on elder care and legal challenges, is preferred.
• Ability to develop and implement personalized care plans, addressing long-term care and estate planning needs.
• Proven track record of collaborating effectively with clients and community resources to ensure comprehensive support.
• Strong communication skills, enabling you to offer expert advice and empower clients with knowledge and confidence.
• Proficiency in maintaining accurate and confidential client records, ensuring seamless service delivery.
• Ability to contribute valuable insights and strategies during team meetings and case reviews.
• Commitment to staying informed on industry trends and legal updates, continuously enhancing your expertise and service offerings.
