Nurse Practitioner

In this role, you will deliver advanced clinical assessments, develop and manage individualized care plans, and collaborate closely with an interdisciplinary team to help patients maintain independence and improve quality of life at home. As a key clinical leader in the field, you will conduct in‑home visits, perform comprehensive health evaluations, diagnose and treat acute and chronic conditions, and adjust treatment plans in response to changing patient needs. You will order and interpret diagnostic tests, prescribe medications within your scope of practice, and provide evidence‑based interventions that support safe recovery, symptom management, and long‑term wellness. Working in the Healthcare & Medical Services department at CenterWell Home Health, you will coordinate care with physicians, registered nurses, therapists, social workers, and caregivers to ensure a seamless, holistic approach to patient support. You will educate patients and families on disease management, medication adherence, home safety, and preventive care, empowering them to actively participate in their own health decisions. Documentation of all clinical encounters will be completed accurately and promptly in the electronic medical record, aligning with regulatory standards, payer requirements, and company policies. You will monitor outcomes, identify potential risks, and proactively implement interventions that prioritize patient safety and reduce avoidable hospitalizations.
