This primary care clinic is part of a well-established healthcare network dedicated to patient and family-centered care. The facility offers a collaborative team environment, on-site services including Physical Therapy, Diabetes Education, Weight Loss, Path/Lab, Radiology/Imaging, ultrasound, and mammogram, as well as outreach services spanning Mental/Behavioral Health, Podiatry, Orthopedic Medicine, General Surgery, and OB/GYN.

Opportunity:
Physician – Family Medicine

Job Details:

• 1.0 FTE, full-time opportunity with 36 patient contact hours per week
• Clinic hours Monday through Friday, 8:00 AM – 12:00 PM and 1:00 PM – 5:00 PM
• Average of 16-20 clinic patients per day
• Collaborate with another Family Medicine physician and 2-3 Nurse Practitioners as part of a dedicated care team
• Call coverage limited to phone call only (nursing home questions and after-hours critical lab call backs) on a 1:4 rotation with other local providers
• Electronic Medical Record system: Epic
• On-site services include Physical Therapy, Diabetes Education, Weight Loss (Non-Surgical), Path/Lab, Radiology/Imaging, ultrasound, and mammogram
• Contracted with local public schools for Sports Medicine coverage and general Health and Well-Being services
• Outreach services include Mental/Behavioral Health, Podiatry, Orthopedic Medicine, General Surgery, and OB/GYN
• Optional inpatient rounding at a local hospital located within a mile of the clinic; nursing home rounds also included
• Board Certified or Board Eligible in Family Medicine required
• Serves as a Care Team leader utilizing established care processes to achieve patient care objectives and provide continuity of care
