At Longleaf Hospice and Palliative Care, we integrate and address the diverse, seen and unseen needs of patients and their families.As a Weekend RN, you will:Complete an initial assessment of patient and family to determine hospice care needs; provide a complete physical assessment and history of current and previous illnessUse health assessment data to determine nursing diagnosisTriage calls and assist patient/primary caregiver with problem resolution; counsel the patient and family in meeting nursing and related needsMake visits for symptom management and support as neededModify patient treatment plans as indicated by patients' responses and conditionsMaintain current, accurate, detailed reports in accordance with policyMonitor, record and report symptoms and changes in patients' condition to the IDGProvide health care instructions to the patient as appropriate per assessment and planObtain and receive physician orders as requiredPronouncement of patientsOther duties as assignedRequirementsCurrent license to practice as a registered professional nurse in GeorgiaCurrent and continued CPR certification2 years of recent hospice experienceGood verbal and written communication and problem solving skillsResponsible valid driver's licenseReliable, insured, automobile for making home visits required
