Position: Rehabilitation Specialist
Department: Psychiatric Rehabilitation Program (PRP)
Reports To: Program Director / Clinical Supervisor
Employment Type: Full-Time / Part-Time, as applicable

Position Summary

The Rehabilitation Specialist is responsible for providing clinical and rehabilitative support to individuals receiving services through the Psychiatric Rehabilitation Program (PRP). The Rehabilitation Specialist works closely with Community Support Workers (CSWs), caseworkers, patients, and other members of the treatment team to ensure that patients receive appropriate, person-centered, recovery-oriented services.

The Rehabilitation Specialist provides clinical oversight, guidance, and support to assigned caseworkers while monitoring patient progress and ensuring that services and documentation are consistent with the patient's treatment plan and applicable program requirements.

Essential Duties and Responsibilities

1. Patient Support and Clinical Oversight

• Provide rehabilitation support and guidance to patients receiving PRP services.
• Review patients' needs, goals, progress, strengths, and barriers to recovery.
• Monitor patient progress toward established treatment plan goals and objectives.
• Provide recommendations and interventions designed to promote independence, stability, community integration, and improved functioning.
• Coordinate with caseworkers and other members of the treatment team regarding patient needs and progress.
• Identify changes in patient needs or functioning and communicate concerns to the appropriate clinical staff.

2. Monthly Patient Video Visits

• Conduct a video call with patients assigned to the Rehabilitation Specialist's caseworker team at least once per month.
• Use the monthly video visit to assess patient engagement, progress, current needs, and barriers to achieving treatment goals.
• Provide appropriate feedback, recommendations, and follow-up based on the patient's needs.
• Document each patient contact in accordance with organizational policies, treatment requirements, and applicable documentation standards.
• Communicate significant concerns or changes in a patient's condition to the appropriate supervisor or clinical provider.

3. Caseworker Supervision and Support

• Provide supervisory meetings with assigned caseworkers once or twice per month , depending on program needs.
• Review caseworker caseloads, patient progress, treatment-plan goals, and service delivery.
• Provide guidance regarding appropriate interventions, patient engagement, community resources, and rehabilitation strategies.
• Assist caseworkers in addressing challenging cases and identifying appropriate solutions.
• Review caseworker documentation for completeness, accuracy, timeliness, and consistency with patient treatment plans.
• Provide coaching and constructive feedback to support caseworker performance and professional development.
• Escalate significant clinical, documentation, compliance, or patient-safety concerns to the appropriate supervisor.

4. Treatment Planning and Documentation

• Assist with the development, review, and updating of patient treatment plans.
• Ensure rehabilitation services are aligned with identified patient goals and objectives.
• Review progress notes and other required documentation for accuracy and consistency.
• Maintain timely and accurate documentation of patient contacts, supervisory meetings, consultations, and clinical recommendations.
• Ensure documentation complies with organizational policies, applicable Maryland regulations, and program requirements.

5. Care Coordination

• Collaborate with providers, caseworkers, therapists, psychiatrists, patients, families, and community resources when appropriate.
• Participate in treatment team meetings and case consultations as needed.
• Assist in coordinating services and referrals to address patient needs.
• Promote continuity of care and communication among members of the patient's treatment team.

6. Quality Assurance and Compliance

• Support compliance with organizational policies, PRP requirements, applicable COMAR regulations, and accreditation standards.
• Assist in identifying documentation or service-delivery deficiencies and recommend corrective actions.
• Participate in quality improvement activities, audits, training, and program development as assigned.
• Maintain confidentiality and protect patient information in accordance with HIPAA and organizational policies.

Required Qualifications

• Bachelor's degree in Psychology, Social Work, Counseling, Rehabilitation, Human Services, or a related field; master's degree preferred where applicable.
• Experience working with individuals with mental health, behavioral health, or psychiatric rehabilitation needs.
• Experience providing case management, rehabilitation, counseling, care coordination, or related services.
• Strong understanding of recovery-oriented and person-centered care.
• Ability to provide guidance, coaching, and supervision to caseworkers.
• Strong written and verbal communication skills.
• Ability to work independently while collaborating effectively with a multidisciplinary team.
• Proficiency with electronic health records and documentation systems.
• Ability to conduct patient and staff meetings through secure video-conferencing platforms.

Preferred Qualifications

• Experience working in a Psychiatric Rehabilitation Program (PRP) or community-based behavioral health program.
• Experience supervising or providing clinical oversight to caseworkers or direct-care staff.
• Knowledge of Maryland behavioral health regulations and documentation requirements.
• Familiarity with ICANotes or other behavioral health electronic health record systems.
• Knowledge of community resources and behavioral health services.

Key Performance Expectations

The Rehabilitation Specialist is expected to:

• Conduct required monthly video visits with assigned patients.
• Conduct caseworker supervisory meetings once or twice per month.
• Provide timely clinical and rehabilitative guidance to assigned caseworkers.
• Ensure patient needs and treatment-plan goals are appropriately addressed.
• Monitor documentation quality and completion.
• Maintain professional, ethical, and confidential interactions with patients and staff.
• Communicate concerns promptly and participate actively in the patient's overall care coordination.
• Support a person-centered, recovery-oriented, and trauma-informed approach to service delivery.
