  • Vision insurance
Location: Delaware, Marion, and surrounding areas
Employment Type: Full-Time

About the Role

As a Tranquility Hospice STNA/Hospice Aide, you will play an important role in providing compassionate, hands-on care to patients and supporting their comfort, dignity, and quality of life.

The Hospice Aide provides personal care and assistance to hospice patients under the direction and supervision of the RN/Case Manager and in accordance with the patient's individualized Hospice Aide Plan of Care.

Following the assigned care plan and completing accurate, timely documentation are essential responsibilities of this position. Care must be provided exactly as directed, and all required documentation must clearly reflect the services provided, observations made, and any changes or concerns identified during the visit.

Being of service to others is more than a career; it is a calling. Tranquility Hospice is committed to supporting our employees while creating a positive, team-oriented work environment centered on compassionate end-of-life care.

Position Responsibilities

Following the Assigned Care Plan

  • Follow the patient's assigned Hospice Aide Plan of Care and all patient-specific instructions provided by the supervising RN/Case Manager.

  • Perform only the care and services authorized within the assigned Plan of Care, applicable regulations, training, and organizational policy.

  • Review assigned care instructions prior to providing services and ensure all required tasks are completed as directed.

  • Immediately communicate any barriers to completing the assigned care plan to the RN/Case Manager.

  • Do not independently add, remove, or change patient-care tasks without direction from the appropriate supervising nurse.

  • Maintain consistency with the patient's current Plan of Care throughout each visit.

Personal Care

  • Provide personal care to patients in accordance with the individualized Hospice Aide Plan of Care and instructions provided by the supervising RN.

  • Assist patients with activities of daily living, including bathing, grooming, dressing, toileting, oral care, feeding, and other authorized personal-care needs.

  • Assist with ambulation, positioning, transfers, mobility, and passive range-of-motion activities as directed by the Plan of Care.

  • Assist with meal preparation and other appropriate activities necessary to support the patient's care and comfort.

  • Provide care in a manner that promotes patient dignity, privacy, comfort, safety, and independence whenever possible.

Documentation

  • Complete all required documentation accurately and promptly following each patient visit.

  • Ensure documentation clearly reflects the care and services actually provided in accordance with the assigned Hospice Aide Plan of Care.

  • Document personal care completed, patient observations, patient response to care, changes in condition, refusals of care, incomplete tasks, concerns, and other required clinical information.

  • Ensure documentation is complete, accurate, relevant, and submitted within required organizational timeframes.

  • Never document care or services that were not actually provided.

  • Promptly notify the RN/Case Manager when an assigned task cannot be completed, a patient refuses care, or the patient's condition or needs have changed.

  • Understand that accurate documentation is a vital component of patient safety, continuity of care, regulatory compliance, and communication with the interdisciplinary team.

Observation & Reporting

  • Observe patients for changes in physical condition, behavior, comfort, functional ability, skin condition, appetite, elimination, or other needs.

  • Promptly report changes in the patient's condition, concerns, or unusual observations to the RN/Case Manager.

  • Report any concerns related to patient safety, caregiver support, or the patient's environment to the appropriate clinical team member.

  • Document reported changes and concerns accurately and in accordance with organizational procedures.

  • Follow patient-specific instructions established by the supervising RN and interdisciplinary Plan of Care.

Patient & Family Support

  • Provide compassionate emotional support and reassurance to patients and caregivers within the scope of the Hospice Aide role.

  • Reinforce education previously provided by the RN or other appropriate clinical professional when directed.

  • Support patients and caregivers throughout the hospice journey while maintaining appropriate professional boundaries.

Care Coordination

  • Communicate regularly with the RN/Case Manager and other appropriate members of the interdisciplinary team.

  • Follow all updates or changes to the Hospice Aide Plan of Care as communicated by the supervising RN.

  • Participate in care coordination and staff communication as required.

  • Report discrepancies between the assigned Plan of Care and the patient's current needs to the RN/Case Manager rather than independently changing the care provided.

Technology

  • Utilize the Electronic Health Record (EHR), email, computers, mobile devices, and other technology required for clinical documentation and communication.

  • Complete required electronic documentation accurately and within established deadlines.

Qualifications

  • Successful completion of an approved Nurse Aide Training and Competency Evaluation Program (NATCEP) and applicable competency evaluation requirements.

  • Current listing in good standing on the Ohio State Nurse Aide Registry.

  • Minimum of one year of STNA/nurse aide experience in hospice, home health, long-term care, or another applicable healthcare setting preferred.

  • Hospice experience preferred.

  • Demonstrated ability to consistently follow assigned care plans, written instructions, and clinical direction.

  • Strong attention to detail with the ability to complete timely and accurate documentation.

  • Ability to recognize and appropriately report changes in a patient's condition.

  • Compassionate and professional approach to caring for patients with serious or terminal illnesses.

  • Strong organizational and time-management skills.

  • Ability to communicate effectively both verbally and in writing.

  • Ability to work independently in patient homes while remaining accountable to the supervising RN/Case Manager and interdisciplinary care team.

  • Valid driver's license and access to a reliable, insured automobile.

Job Conditions & Physical Requirements

  • Position requires frequent travel throughout Delaware, Marion, Hardin, Wyandot, Morrow, Union, and surrounding service areas.

  • Majority of patient visits are expected to be within Delaware County, based on current patient census and organizational needs.

  • Must be able to travel to and access patients' homes, facilities, and other care environments.

  • Physical activities may include walking, sitting, standing, bending, stooping, reaching, positioning patients, and assisting with mobility.

  • Must be able to lift up to 50 pounds.

  • Must be able to perform the essential functions of the position, with or without reasonable accommodation.
Our Hospice Mission

Tranquility Hospice began with the story of losing a beloved family member and experiencing firsthand the challenges that can occur when hospice care does not meet a family's expectations. Following that life-altering experience, we chose to combine our personal and professional expertise in the medical field to create a different kind of hospice experience in Delaware, Ohio.

We pride ourselves on being a freestanding hospice provider in our community that is not tied to another healthcare system. Everything we do is guided by our commitment to providing patients and their families with the compassionate, high-quality end-of-life care they deserve.

The Tranquility Difference

At Tranquility Hospice, we move at a different pace. Full-time employment does not mean routinely working hours off the clock. We recognize that when healthcare professionals feel rushed or unsupported, it becomes more difficult to provide the level of care our patients deserve.

Our commitment to excellent patient care begins with creating a positive work environment for our employees. We are dedicated to supporting our team, encouraging open conversations about how we can continuously improve, and maintaining a safe and respectful workplace.

If this approach to hospice care sounds meaningful to you, we would love to have a conversation about how you could become part of the Tranquility family.

Equal Employment Opportunity

Tranquility Hospice LLC is an Equal Opportunity Employer and Drug-Free Workplace.

Tranquility Hospice LLC / Tranquility Health Care LLC complies with applicable federal civil rights laws and does not discriminate based on race, color, religion, sex, including pregnancy and gender identity, national origin, political affiliation, sexual orientation, marital status, disability, genetic information, age, membership in an employee organization, retaliation, parental status, military service, or any other characteristic protected by applicable law.
