Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficient in HomeCare HomeBase and Oasis within the last 6 months Description: Coordinates and directs the provision of home nursing care according to physicians' orders. Applies the nursing process competently based on agency policies and procedures. Responsible to Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits and periodic office meetings are required. Must have reliable transportation, a current driver’s license, and agency-required liability insurance. Average number of patients per day is 5-8. The contractor will be assigned between 5-10 patients per day. Each visit and activity has a different point value, and contractors must meet a minimum of 30 points per week. Types of cases include Start of Care, Wound Care, and follow-up visits. Must be comfortable with travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. Must be open to weekend shifts and on-call rotations as needed. All documentation must be completed within 24 hours of the appointment. Delayed and incomplete documentation will impact billing and follow-up patient care. No guaranteed hours are available with the contract. The clinician must accept every patient on their schedule regardless of distance. All travelers must be proficient with HomeCare HomeBase EMR and Oasis. Must be willing to float to another nearby clinic if patient census is low.
