Empowering Home Health RN | Care Coordination & Advocacy

Interim HealthCare Inc. is seeking a Home Health Registered Nurse in Illinois. The role focuses on assessing home care needs, coordinating services, and educating patients and families on care plans. The RN will document progress, communicate with other providers, and support transition planning for patients returning home. Qualified candidates hold an IL RN license, CPR/BLS, and 2+ years in home health or similar settings.
