Speech-Language Pathologist

Effectively screens, evaluates, and rehabilitates patients with communication, cognitive, or swallowing disorders. Develops appropriate plans of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Modifies services based on patient evaluations and makes referrals related to outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers regarding speech disorders. Communicates effectively with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services rendered per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided in compliance with all regulatory and clinical requirements. Participates in required meetings including patient care conferences and rehabilitation meetings. Engages in facility and company-required trainings, in-services, and conferences. Able to obtain necessary DME or supplies per regulatory and clinical practice requirements. Supervises and supports CFY or Student Speech-Language Pathologists. Adheres to ASHA and state required practice acts, code of conduct, and ethics during professional practice.

Experience: 1 year of SLP experience or Level II experience for new graduates License: Massachusetts State License Certifications: CCC certified SLP Must-Have: LTC or rehab experience
