Locations: Northern Nevada Including Reno, Sparks, Carson City, and Surrounding Areas

Make a difference where it matters most at home. Were looking for a dedicated Physical Therapist (PT) to join our compassionate home health team. Help patients regain mobility, independence, and quality of life all from the comfort of their own homes.

Main Responsibilities:

• Conduct patient assessments, create and update care plans, and establish clear, measurable goals.
• Teach patients and their families exercises and self-care techniques that enhance mobility and promote safety.
• Complete home safety evaluations and provide recommendations for modifications when necessary.
• Work closely with physicians, nurses, and the interdisciplinary team to support high-quality patient outcomes.
• Ensure all visit notes and documentation are completed accurately and on time.
• Join case conferences and regularly update the care team on patient progress.
• Help patients and their families understand care goals, expectations, and ongoing progress.

Qualifications:

• Current Physical Therapist (PT) license in Nevada (required).
• Minimum 1 year of home health physical therapy experience (preferred).
• Strong clinical, communication, and problem-solving skills.
• Ability to work independently and manage a flexible caseload.
• Valid drivers license and reliable transportation.
