Care Manager LCSW

Care Manager, LCSW Providence Little Company of Mary Medical Center San Pedro, a nationally accredited medical center, is seeking a licensed LCSW to serve as Care Manager in its Psychiatry Department. Position: Full‑time, 8‑hour day shift Location: 1300 W 7th St, San Pedro, CA The successful candidate will be placed within the wage range of $43.16 – $68.21 per hour, with potential for additional compensation such as shift differentials, standby/on‑call pay, overtime, bonus opportunities, and other incentives. Responsibilities Provide comprehensive care management and transitional care for psychiatric patients, coordinating with patients, caregivers, nurses, physicians, social workers, payers, and community resources. Facilitate communication and support decision‑making, respecting patients’ right to self‑determination and addressing social determinants of health. Collaborate with interdisciplinary team to develop and implement discharge plans and post‑discharge support. Required Qualifications Master’s degree in Social Work. Current California Clinical Social Worker License. Compliance with LosAngeles County Department of Mental Health guidelines for LPS designation; maintain designations in current status. Designated as Behavioral Health Clinician within 18 months of hire to perform 5150 evaluations. CPI (nonviolent crisis intervention) certification within 1 year of hire. At least 2 years of experience in a healthcare setting with a psychiatric/mental‑health patient population, preferably inpatient. Proficient computer skills. Preferred Qualifications Experience in discharge planning.
