RN Quality & Care Liaison: Kidney Health Improvement
Healthmap Solutions
Healthmap Solutions is seeking an RN Quality Practice Advisor to coordinate performance improvement activities that educate providers and enhance health outcomes for Healthmap members. The role partners with the Care Navigation team to strengthen the Kidney Health Management program. You will build relationships with clinical sites, review provider data to identify improvement opportunities, educate on NCQA standards and HEDIS, and lead process changes. #J-18808-Ljbffr Healthmap Solutions
