What you’ll be doing

• Supporting members through their recovery journey.
• Helping coordinate care and connect members to the right services.
• Building strong, trusting relationships with the people you serve.
• Partnering with clinical leaders and care teams to support member success.

What you’ll need

• High school diploma or equivalent.
• At least 2 years of experience in case management, care coordination, or a similar role.
• Strong communication, empathy, and organization skills.
• A genuine desire to help people achieve recovery and stability.
• Experience using Electronic Health Records (EHR).
• Reliable internet access (50M download / 10M upload) and strong WiFi if working remote.
• Must meet state-specific background screening and pre-employment requirements (which may include fingerprinting, drug testing, CPR/First Aid, or health screening).
• Valid driver’s license and access to a vehicle (if role includes community travel).
