Pay: $33.65/hr

The Senior Social Worker plans, organizes, and supervises the Social Services program. This position provides leadership and oversight of social services operations while ensuring compliance with federal and state regulations, Centers for Medicare & Medicaid Services (CMS) requirements, and facility policies and procedures.

The Senior Social Worker oversees resident assessments, care planning, discharge planning, staff supervision, and documentation while maintaining a personal caseload of assigned veterans and members. This role works closely with residents, families, interdisciplinary healthcare teams, and community partners to promote resident rights, safety, dignity, and quality of care.

Essential Duties and Responsibilities

• Plan, organize, and supervise the facility’s Social Services program.
\n
• Ensure timely completion of resident assessments, documentation, and care plans in accordance with applicable federal and state regulations.
\n
• Ensure compliance with the Preadmission Screening and Resident Review (PASRR) process.
\n
• Supervise, train, and support Social Services staff to achieve departmental goals and objectives.
\n
• Assign, review, and evaluate staff workloads and job performance.
\n
• Conduct monthly one-on-one coaching sessions and annual performance appraisals.
\n
• Address employee performance concerns and recommend appropriate disciplinary actions.
\n
• Participate in staff interviews, recruitment, and selection.
\n
• Oversee clinically appropriate discharge planning to ensure safe transitions of care and effective communication with residents, families, and other stakeholders.
\n
• Monitor Medicare notices to ensure timely delivery to beneficiaries in accordance with federal requirements.
\n
• Complete consultation requests and referrals to Veterans Affairs (VA) specialists as appropriate.
\n
• Advocate for residents and ensure that veterans and members are treated in accordance with established Resident Rights.
\n
• Collaborate with residents, families, healthcare providers, and interdisciplinary teams to coordinate services and address resident needs.
\n
• Assist with departmental budget projections, monitor expenditures, and maintain compliance with budget parameters.
\n
• Maintain an assigned caseload and provide direct social services to veterans and members.
\n
• Attend and participate in departmental meetings, interdisciplinary care conferences, and facility committees.
\n
• Maintain accurate, confidential documentation and comply with applicable privacy requirements.
\n

Knowledge, Skills, and Abilities

Knowledge of:

\n
• CMS requirements, state regulations, and long-term care standards.
\n
• Social services programs, social work principles, and case management practices.
\n
• Geriatric psychiatric conditions, age-related disease processes, and the effects of psychotropic medications.
\n
• Physical and chemical restraint regulations and applicable resident safety standards.
\n
• Community resources and governmental programs affecting veterans and older adults.
\n
• Long-term care billing standards and the Minimum Data Set (MDS) process.
\n

Minimum Qualifications

\n
• Master’s degree in Social Work (MSW) required.
\n
• Active Licensed Master Social Worker (LMSW) credential issued by the Arizona Board of Behavioral Health Examiners, as required for placement in the position.
\n
• At least one year of supervisory experience in a healthcare setting.
\n
• Experience in long-term care preferred or required according to the applicable hiring criteria.
\n

Pre-Employment Requirements

\n
• Ability to obtain and maintain a valid Arizona Department of Public Safety (DPS) Fingerprint Clearance Card.
\n
• Current negative tuberculosis (TB) screening, including a Mantoux skin test within the required timeframe, in accordance with facility policy.
