Senior Living Physical Therapist (Hybrid/Telehealth)
Reliant Rehabilitation
Reliant Rehabilitation is seeking a licensed physical therapist to provide patient-focused services across Assisted Living, Independent Living, and Memory Care settings. You will screen, evaluate, and treat patients, develop plans of care, and manage discharge planning while upholding Reliant Rehabilitation's standards.
The role includes supervising PTAs and Rehab Techs, coordinating with interdisciplinary teams, and ensuring compliance with OBRA and state guidelines.
