Key Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Design functional treatment programs that support effective patient return to community activities and progress patients toward discharge within an established length of stay
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under valid physician orders and within insurance approvals
• Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific state practice acts and regulatory requirements, and actively pursue professional development through continuing education and professional organizations

Required Qualifications

• Active Physical Therapist license in good standing, meeting all state-mandated continuing education requirements
• Minimum of one year of clinical experience required — new graduates will not be considered
• Demonstrated ability to apply clinical reasoning to develop appropriate treatment plans and goals
• Commitment to patient and employee safety standards, including safe patient handling and infection control practices
• Ability to perform in an ethical manner consistent with discipline-specific professional codes of conduct

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Familiarity with performance improvement methodologies and quality initiatives
• Experience supervising or mentoring physical therapy students
