Requirements: Must have 1 year of inpatient case management experience

Responsibilities:

• Provide discharge planning and referrals for inpatient hospital patients.
• Collaborate with interdisciplinary teams, including physicians and social workers.
• Manage a caseload of at least 20 patients, including discharge to home, community, or SNF.
• Obtain housing, shelter packets, and specialized referrals such as Hemodialysis and Wound Vac.
• Utilize electronic medical records (Allscripts) to document patient progress and discharge plans.
