Care Team Case Manager

CARE Case Manager 3 Are you experienced managing student caseload using a systems-based approach to support success and well-being? Do you have experience monitoring student care action plans, providing follow-up, and ensuring accountability for recommended supports? Do you have experience acting as a liaison between referrers and the student support or care team? Have you provided training or given presentations to help increase awareness of student support resources?

Position Summary The CARE Case Manager 3 supports the Student Care and Outreach Team ("Care Team") in managing cases involving students of concern, with an emphasis on promoting student well-being and success. This role includes initiating cases, coordinating follow-up actions, documenting team decisions, and monitoring student progress through established action plans. The position is responsible for managing daily Care Team access points and providing status updates on active cases. Will also serve as a liaison to faculty, staff, parents, and other stakeholders seeking consultation or assistance from the Care Team. Additionally, this role may support Temporary Conditions and Withdrawal Services by coordinating accommodations and assisting students in navigating short-term or hardship-related academic adjustments. The position contributes to campus-wide awareness of Care Team services through training, presentations, publications, and data reporting.

Key Responsibilities Manage a caseload of students using a system-based approach to support success and well-being. Triage Care Team cases and gather information to make recommendations. Meet with students to provide support, build resilience, and connect them to resources. Open and manage cases; coordinate interventions; support care plans; track progress in Maxient. Close cases, ensuring all plans and follow-up actions are completed. Coordinate projects related to grants, basic needs, and best practices. Provide outreach and training to the campus community to increase awareness and support students. Develop solutions and accommodations for students impacted by injuries, surgery, or short-term conditions affecting daily functioning. Support students navigating leaves of absence or retroactive withdrawals due to hardship. Create trainings, publications, and data reports to educate the university community on Care Team referral process and supporting students in distress. Track trends using case management data to identify gaps and growth areas. Develop and administer surveys to identify process improvement measures.

Job Related Minimum Required Education and Experience - Requires a bachelor's degree in a job related field and 2 or more years of job related experience, which may be substituted by an equivalent combination of job related certification, training, education, and/or experience.

Required Certification, Licensure/Other Credentials

Preferred Qualifications - Experience with Student Care teams, Title IX cases and trauma-informed interviewing practice, maxient electronic database and case management. The ability to balance multiple competing priorities. Experience with Student Conduct and Honor Code cases. Pending graduates will be considered if they will be able to obtain their degree by the date of employment.

Knowledge/Skills/Abilities - Able to organize data to identify problems and develop and implement realistic solutions. Communicate effectively orally and in writing.

Job Duties

Job Duty - Assist students who are referred to the Student Care and Outreach Team Support an ongoing caseload of students who benefit from a systems approach to student success, wellness, and retention. Triage Care Team cases daily with supervisor and gather information regarding referred students to assist in the development of recommendations. Coordinate communication and recommendation completion with students referred to Counseling & Psychology Services; consult with Counseling and Psychology Services on scheduling assessments and assisting students with meeting counseling requirements. Conduct meetings (soft interventions) with students referred for lower levels of distress to assist with students' resiliency and connections to resources, and counseling pre-assessment meetings with students scheduled for counseling assessments. Enhance and further customize the network of support services for students. Monitor action plans and provide schedule check-ins with students to ensure follow-through with recommendations. Follow up when appropriate with those who can support students affected by a referred student's behavior, including Residence Life staff and faculty members.

Essential Function - Yes Percentage of Time - 40

Job Duty - Assist the Student Care and Outreach Team with case management Open cases, schedule, and coordinate interventions, assist in care plan development, and track student persistence in completion of the team's recommendations using the Maxient database. Close cases, assuring all case management plans have been addressed. Manage communication channels daily, including Care Team email, Care Team phone line, drop-in requests, and questionnaire forms. Coordinate special projects and initiatives for grant proposals, basic needs support, and best practices initiatives in the field of higher education case management. Deliver outreach and training for the campus community to increase awareness of the Care Team's services and ways to assist students. Serve as the Referrer Liaison for reporters who document needing to speak to a member of the Team. May supervise a graduate student from the Higher Education and Student Affairs master's program and/or a Master of Social Work intern Update and assist in the preparation of the agenda for weekly Care Team meetings. Generate statistical reports relevant to the Team and identify trends for the university community. Keep detailed notes of interactions with students, confidential reports, parental, and campus partner communication.

Job Duty - Assist with coordinating the functions of Temporary Conditions and Withdrawal Services Create solutions to meet students' needs that are a result of injuries, surgery or short-term conditions that substantially limit their major daily life activities. Support students who need to take time away from school or need to retroactively withdraw due to hardship.

Essential Function - Yes Percentage of Time - 10

Job Duty - Assist the Care Team with education, training, and outreach Create trainings, publications, and data reports to educate the university community on Care Team referral process and supporting students in distress. Track trends using case management data to identify gaps and growth areas. Develop and administer surveys to identify process improvement measures. Other duties as assigned through the Office of the Dean of Students.
