Role Description The Care Coordination Manager is a full-time, on-site role based in Friendswood, TX. This role is responsible for overseeing the coordination of medical and non-medical homecare services, ensuring that client care plans are accurately developed, implemented, and updated as needs change. Day-to-day responsibilities include assessing scheduling and matching caregivers to clients, monitoring service delivery, and maintaining clear communication among clients, families, caregivers, and clinical staff. The Care Coordination Manager will track documentation, support regulatory and policy compliance, and collaborate with branch leadership to maintain quality standards and operational efficiency. This position also involves resolving service issues, supporting caregiver engagement and performance, and contributing to continuous improvement of care coordination processes. Qualifications Care management and coordination skills Scheduling and staffing coordination abilities, such as matching caregivers to client needs, managing calendars, and handling last-minute changes effectively. Strong communication and interpersonal skills for interacting with clients, families, caregivers, and clinical professionals in a clear, respectful, and collaborative manner. Organizational and administrative skills, including attention to detail, accurate documentation, and proficiency with office software and care management systems. Knowledge of home healthcare operations, basic regulatory requirements, and safety standards related to medical and non-medical in-home care. Demonstrated problem-solving and conflict resolution skills, with the ability to make sound decisions in time-sensitive situations.
