About the Position

Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 16:30 Experience: Minimum of 2 years in acute care License: Active Registered Nurse License Certifications: Must have relevant certifications Must-Have: - Acute care experience - Familiarity with case management principles - Experience with HMOs, IPAs, advance directives, and protective services process Description: The position requires the candidate to work every other weekend. Candidates with 2 years of acute care experience are preferred. First-time travelers will be accepted based on clinical approval. There is no call required for this position, but floating between like units may be necessary. Local candidates are welcome to apply. All requests for time off should be included during the submission process. The position utilizes Epic for charting. Travelers will be supported in finding housing if needed. The role involves working with a diverse patient population, including adolescents, adults, pediatrics, and geriatrics. The daily caseload can include up to 25 patients. Interdisciplinary support is available. The candidate should be current in hospital case management experience. References are required from the past year of employment. Specific skills in discharge planning and appeals and denials are necessary. Providers may be required to work holidays and commit to floating between facilities. Compliance with HIPAA guidelines is essential. This role involves care coordination, benefits eligibility assessment, and needs assessment. Experience with CMS and National Safety Goals is highly valued. The successful candidate must demonstrate the ability to manage complex case loads effectively.
