Travel RN Case Manager

Travel RN Case Manager position available. Candidates must have experience in acute care case management. Responsibilities include utilization review, discharge planning, and transitions of care. Home health and skilled nursing facility setups are part of the role. Successful candidates will work collaboratively with teams, including physicians and interdisciplinary teams. The unit includes 42-50 beds with a patient ratio of 2:2. Computer documentation systems are utilized for patient records. The dress code is business casual. Candidates should be aware of possible weekend rotations. Unit policies and guidelines provide a structured work environment. There are opportunities for rounding with various teams. This position offers a chance to improve patient care and outcomes.
