Position Purpose: All KHI employees are expected to perform their respective tasks and duties in such a way that supports KHIs vision to be among the kindest, highest quality health systems in the country. Provide intensive oncology pharmaceutical care services in collaboration with other health professionals. Promote high quality, evidence-based, cost-effective oncology pharmaceutical care, managing patient drug therapy, provide patient education and perform duties in the KRMC Cancer Center and other clinical areas as required and assigned.

Key Responsibilities:

• Focus clinical pharmacy activities to establish and foster KRMC Pharmacy oncology services, serving as a resource and leader to physicians and other caregivers.
• Supervises and provides oncology clinical pharmacy activities, including but not limited to antineoplastic, cancer pain, and general oncology pharmaceutical care for our Cancer Center and KRMC Inpatient care areas.
• Assist in activities to promote and deliver effective pain management for Cancer Center and other patients.
• Identify, resolve, and prevent potential and actual medication-related problems including: untreated indications, improper drug selection, sub-therapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medication without indication.
• Initiate, adjust, monitor, and recommend changes in drug therapy based on information obtained through collaborative practice, patient interviews, objective monitoring parameters, review of drug profiles and the patient medical record.
• Coordinate and interpret laboratory monitoring of drug therapy, including drug dosing to established protocols and best practice, serum drug levels, electrolytes, hematology and metabolic parameters, along with other drug-related laboratory testing.
• Provide patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization and patient compliance, detecting and/or minimizing ADRs, eliminating unnecessary drug use and duplication.
• Focus clinical pharmacy activities to decrease overall patient healthcare costs by effective pharmacotherapy, leading to efficient use of Cancer Center and KRMC resources resulting in overall better patient clinical outcomes.
