Position Overview PRN Physical Therapist (PT) - Flexible Hours, Great Team, Meaningful Work! Community Physical Therapy (CPT) is expanding our Continuum of Care (COC) program, and we’re looking for passionate PTs to join us! Work with residents right where they live – providing both outpatient and home care therapy in Alden communities. Responsibilities Provide outpatient and home care therapy to residents in Alden communities. Maintain accurate and timely documentation using TheraOffice & Homecare Homebase. Collaborate with a multidisciplinary team to support residents’ functional goals. Qualifications Licensed Physical Therapist with current state certification. Experience in outpatient and/or home care settings. Flexibility to accept PRN shifts within the schedule (Mon‑Fri, 7am‑6pm). Strong communication and teamwork skills.
