We CARE for our patients like they are our own FAMILY. Christ Hospital Home Health Schedule: 3 weekdays 1pm to 9pm, on‑call once a month and 2 holidays per year. Territory includes Hamilton, Trenton, Avondale, Fairmont, Oxford, Ross, Sharonville, WestChester, Colerain, ForestPark, Queen City, and Westwood areas. How You’ll Make a Difference As a Registered Nurse (RN) you will provide one‑on‑one patient care and work at the top of your license. You will use your leadership skills to coordinate care and provide home‑based nursing care for patients as directed by an attending physician. You will be part of an interdisciplinary team focused on compassionate, quality care and positive outcomes. How You’ll Work You'll provide skilled nursing care in patients’ homes as directed by the attending physician, manage your time efficiently, and perform all duties within the scope of practice outlined by the state. Major Areas of Responsibility Plan of Care: Observe and monitor patient conditions and perform OASIS assessments to develop an individualized care plan and adjust as needs change. Minimize Patient Risk: Administer medication as prescribed by the physician and help decrease re-hospitalizations by prioritizing visits for high‑risk patients. Continuity of Care: Manage multidisciplinary care and promote continuity with appropriate admissions, transfers, and discharges. Family Educator/Advocate: Educate the patient and family on disease processes using teach‑back methods to ensure understanding. Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaboration: Collaborate with the interdisciplinary team and physicians to ensure optimal care. Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations, and communicate schedule changes to schedulers or supervisors. Operations: Attend in‑service training and mandatory agency meetings. Hard & Soft Skills Compassionate communicator with a positive attitude. Patience when working with patients, families, physicians, and coworkers. Attention to detail is critical as is being observant and following directions. Requirements Registered Nurse with a current license in the state of employment. Minimum one‑year experience as an RN in an acute care setting. Home care experience preferred. Valid driver’s license and auto insurance in your name as a driver. Ability to work 50% of the shift after 5:00PM. Capable of all physical demands.
