Private Duty Nurse LVN - Days/Nights

Role: Licensed Practical / Vocational Nurse (LPN/LVN) providing skilled nursing care in home settings. Responsibilities: Deliver and document patient care according to care plans and physician orders assess patient conditions report changes update nursing care plans educate patients and families and ensure infection control Maintain confidentiality, comply with regulations, and participate in ongoing education Use electronic documentation tools and coordinate care to achieve positive clinical outcomes Qualifications & Skills: Active, unrestricted LPN/LVN license in the state current CPR certification TB test within last 12 months and reliable transportation Preferably six months of clinical experience, with home health experience advantageous Skills include attention to detail, time management, effective communication, problem-solving, and organization Physical ability to lift and reposition patients, travel, and respond to emergencies Additional Info: Must meet state-specific requirements, adhere to infection control standards, and be adaptable to various home environments.
