Registered Nurse - Case Management

Overview The Case Manager Nurse coordinates patient care across settings. They optimize utilization and transitions in acute or home health care. The ideal candidate will have expertise in care coordination and patient advocacy, with experience in hospital or community settings. Key Responsibilities Assess patient needs and develop care plans. Coordinate care with physicians and resources. Advocate for patient access to services. Document care plans in EHR systems like Epic. Monitor patient outcomes. Educate patients on care plans.
