Provide education to patients and their families about therapeutic objectives, discharge planning, and necessary home exercises.

Document evaluations, treatments, and progress meticulously in keeping with facility and regulatory protocols.

Participate in care team meetings and interdisciplinary conferences to ensure seamless care coordination.

Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program.

State licensure as a Physical Therapist.

CPR certification is preferred.

Specialty certifications (e.g., neurological, geriatric) are advantageous but not mandatory.

Prior experience is highly preferred.
