As a nonprofit rooted in community trust, VNA upholds a mission of patient-centered care, reinvesting resources into clinical excellence, family support, and accessible community health services.The Medical Social Worker evaluates the social and emotional factors affecting a patient's illness, care needs, ability to function, and capacity to cope with challenges in daily living with our Home Health and Palliative Care patients. Collaborates with the healthcare team to develop individualized treatment plans and actively participates in patient care conferences.Status: Per DiemEducation/Experience/Skills/Qualifications:Bachelor of Science in Social Work with at least one year of healthcare experience, or a Master's degree in Social WorkAt least one year of social work experience in a healthcare setting, preferably in home healthPossess a current CPR certificationSpecial Requirements:Current California driver's licenseAuto insuranceReliable vehicleJoining VNA means becoming part of a trusted, mission-driven organization with more than 74 years of community-based service.
