Title
Case Management - Care Coordination

Schedule
Weekends: 2 weekends every 6 weeks

Requirements
- Minimum 2 years of clinical experience
- Current clinical work history within the last 2 years
- Required certifications: BLS
- Experience with electronic charting (Epic)
- Travel experience required
- Experience with care coordination and discharge planning
- Knowledge of utilization review and medical necessity

Roles & Responsibilities
- Coordinate patient care and manage discharge planning
- Independently arrange DME (Durable Medical Equipment), SNF (Skilled Nursing Facility) placements, and transport
- Lead family meetings and contribute to treatment planning
- Conduct utilization reviews and manage prior authorizations
- Collaborate with interdisciplinary teams

Compliance / Notes
- Must commit to working 3 of the following 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day
- Pre-approved time off: one day per contract language
- Free parking available
- Orientation: 40 hours
