Home Health Physical Therapist PRN

Location Charlestown by Erickson Senior Living, Charlestown, Maryland. Position Resident-Centered PRN Home Health Physical Therapist. This role focuses on providing holistic care to residents within the community, supporting their physical, spiritual, emotional, and clinical needs. Responsibilities Implement residents’ personalized care plans, emphasizing strengths, preferences, and routines. Provide holistic physical therapy interventions within the continuity of care. Collaborate with the interdisciplinary clinical team to ensure resident needs are met. Maintain accurate documentation to meet reimbursement and regulatory standards. Apply therapeutic techniques appropriate to older adults, including urinary incontinence, vestibular rehab, home safety assessment, movement disorders, and low vision programs. Qualifications Valid and current state Physical Therapy license. Degree and training that meet current Medicare and state requirements. Ability to complete documentation required for reimbursement and regulatory compliance. Therapeutic experience with older adults across multiple diagnostic groups (preferred).
