Schedule: 8:00 AM – 4:30 PM, Monday-Friday or Friday-Monday rotation with weekend coverage

Requirements:

• Active Colorado RN license or ability to obtain
• Minimum 1 year of current Case Management RN experience (required)
• Discharge planning experience strongly preferred
• Emergency Department experience preferred
• Knowledge of palliative care and goals of care conversations is a plus
• Strong communication and interpersonal skills
• Adaptability to work across different units as needed
