\n \nWe are hiring a Physical Therapist (PT) for a Home Health position providing skilled care to patients in their homes in the Colonial Park, PA area. This position is eligible for $20,000 total bonus potential , including a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $10,000 sign-on bonus paid after 30-days of employment! To receive these bonuses, you must be in good standing & meet performance expectations, and in the role and status for which you were hired!

\n \nWhat You’ll Do:

- \n
Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
\n - \n
Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
\n - \n
Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
\n - \n
Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
\n - \n
Work independently, but never alone
\n - \n
Travel to patient homes within assigned territory

PT Qualifications
\n- \n
- \n
Active Physical Therapist, PT license in the state you work
\n - \n
One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
\n - \n
Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
\n
