The RN performs patient assessments, implements physician-ordered care plans, provides patient and caregiver education, coordinates care with healthcare providers, and maintains timely clinical documentation. Essential Responsibilities Skilled Nursing Care Perform comprehensive nursing assessments and reassessments Regularly evaluate patient nursing needs and response to treatment Implement and update the established plan of care Provide skilled nursing services in accordance with physician orders Deliver preventive, restorative, and therapeutic nursing interventions Identify changes in patient condition and take appropriate clinical action Educate patients and caregivers on disease management, medications, and self-care techniques Participate in discharge planning and transition of care activities Care Coordination Communicate patient status changes to physicians and obtain orders as needed Coordinate care with therapists, aides, social workers, and other healthcare professionals Collaborate with community resources to support patient goals and outcomes Advocate for patient needs and quality care Documentation & Compliance Complete accurate and timely clinical documentation Maintain compliance with Medicare regulations and agency policies Follow HIPAA and patient confidentiality requirements Participate in agency quality improvement initiatives Qualifications Graduate of an accredited school of nursing Current Ohio Registered Nurse (RN) license Current CPR certification Valid driver's license Reliable transportation with current automobile insurance Strong assessment, communication, organizational, and problem-solving skills Ability to work independently in a field-based environment Preferred Home Health experience OASIS experience Case Management experience Physical Requirements Frequent walking, standing, bending, and reaching Ability to lift, transfer, or assist patients as necessary Ability to travel throughout the Lancaster and surrounding service area Work Location Field-based position serving patients in: Lancaster, OH Fairfield County Surrounding communities Job Type Per Diem / PRN License/Certification Required: Ohio RN License CPR Certification Driver's License #J-18808-Ljbffr Triad-Home-Health-Services-2
