Job Title: RN Home Health Profession: RN Home Health Specialty: RN Home Health Duration: 17 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year of experience in certified home care and case management License: Current licensure in the state of Massachusetts Certifications: N/A Must-Have: 1 year of certified home care experience 1 year of case management experience Reliable, independent means of transportation for home visits Willingness to work weekends, holidays, and on-call as per departmental needs and rotation Weekend rotations every 6 weeks Description: Provide clinical services to patients from birth to over 100 years in their homes. Assess patient conditions and implement care plans. Provide ongoing education and support to patients and their families. Perform the admissions process and case management responsibilities according to established policies and standards of practice. Utilize thorough and timely electronic documentation and interdisciplinary communications.
