DISEASE INTERVENTION SPECIALIST OUTREACH NURSE
Purpose of Job The purpose of this classification is to provide care and care coordination to people living with HIV, particularly those with high unmet social needs including those experiencing homelessness, and services related to HIV and STI treatment, prevention, and surveillance. Essential Functions: Manages a caseload of individuals with HIV experiencing homelessness and complex medical or social needs, ensuring continuity of care and follow-up. Assesses barriers to care for each client, facilitates enrollment in Ryan White Part B enrollment, and coordinates referrals to supportive services. Collaborates closely with multidisciplinary case managers and partner agencies to optimize client outcomes. Provides direct nursing care in community settings, including encampments and other non-traditional locations. Conducts comprehensive physical and psychosocial assessments; performs HIV and STI testing and treatment, vaccinations, wound care, and other clinical services as needed. Provides occasional clinical coverage for the Communicable and Environmental Disease and Emergency Preparedness (CEDEP) mobile medical unit. Interviews clients regarding health behaviors, risk factors, contacts, and other information relevant to epidemiologic investigation and partner services. Performs phlebotomy and prepares specimens according to clinical protocols. Required Qualifications: Education: Associate's Degree in Nursing Certifications (Required By Hire): Current Registered Nurse licensure in Tennessee Certifications (Required After Hire): ACRN within 2 years of hire Experience: None Preferences: Bachelor’s in Nursing preferred, experience in nontraditional clinical settings desired. Other Requirements: Must possess and maintain a valid TN State Driver's License; must have reliable transportation and maintain automobile liability insurance that meets Tennessee’s financial responsibility law on personal automobile; must be able to enter the homes of clients. Substitutes for Requirements: Any equivalent combination of education, training and experience which provides the requisite knowledge, skills, and abilities may be considered for employment into the position. Background check will be conducted prior to hire. Motor Vehicle Record (MVR) review will be conducted prior to being authorized to drive for county business. Key Knowledge: None Key Skills: Proficient in Windows OS and Microsoft Office. Strong attention to detail and problem-solving skills. Team-oriented with excellent interpersonal and customer service skills. Capable of handling moderately complex tasks. Effective verbal and written communicator. Culturally sensitive and respectful to clients from diverse backgrounds. Self-motivated and able to work independently. Adapts quickly to learn new public health software. Abilities: This position requires the ability to engage in occasional light physical activity. Additional Job Functions: Local and occasional overnights are required. Occasional after-hours/evening work may be necessary. Respond to medical emergencies per KCHD protocol. Respond to and report immediately if called upon by local, regional, or central office supervisors, as part of a coordinated emergency response by the Knox County Mayor’s Office. Ensure regular, punctual attendance during scheduled work hours. Perform other duties as required. Exposure to bloodborne pathogens. Working Conditions: Provides services in varied environments, including residences that may be unsanitary, in poor repair and in neighborhoods with elevated safety risks; adheres to agency safety protocols and exercises sound judgment to minimize personal risk.
