Position: Case Manager RN

Shift: Day 5x8-Hour (08:00 - 16:30)

Job Description

This role requires a Registered Nurse (RN) with significant experience in discharge planning and inpatient acute case management. The Case Manager RN will be responsible for care coordination and care transitions planning throughout the acute care patient experience. The position involves collaboration with physicians, utilization managers, medical social workers, and bedside RNs to ensure the timely progression and transition of patients to the appropriate level of care.

Key Responsibilities

• Conduct initial and ongoing patient assessments, including gathering medical, psychosocial, and financial information.
• Determine risk levels for readmission and refer patients to ancillary supportive services as needed.
• Supervise and lead the healthcare team in developing effective care coordination plans.
• Monitor patient length of stay and facilitate transitions for patients no longer requiring acute care.
• Ensure compliance with Medicare regulations and deliver necessary patient documentation.
• Develop comprehensive transition plans in collaboration with patients, families, physicians, and community resources.
• Address complex clinical and social situations to prevent unnecessary admissions and ensure optimal care transitions.
• Maintain effective communication with the interdisciplinary team to ensure quality patient care.

Qualifications

• Current California RN License required.
• BLS certification is mandatory.
• Minimum of 5 years of RN experience in case management is required.
• Experience in inpatient acute case management is essential.
• Knowledge of CMS and Joint Commission regulations related to care transitions is preferred.

Skills

• Strong care coordination and discharge planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work effectively within a multidisciplinary team.
• Proficiency in electronic health record (EHR) systems, preferably Epic.
