Hospice Field RN Case Manager: Deliver Compassionate Care
MJHS
MJHS Hospice and Palliative Care seeks a Field Case Manager to perform field-based nursing assessments and document visits electronically. You will develop care plans and recommend appropriate nursing care for patients in both community and facility settings. The role includes one weekend per month and a backup On Call 2-3 times monthly, with car and escort support for On Call shifts. Experience in med/surg/community health is preferred. #J-18808-Ljbffr
