What You'll Do Complete physical therapy evaluations in patients' homes Develop individualized plans of care Provide therapeutic exercise, gait, balance, transfer, and mobility training Help patients improve strength, endurance, and functional independence Assess home safety and fall risks Educate patients and caregivers Monitor progress and adjust treatment plans as needed Complete reassessments and discharge planning Coordinate care with the interdisciplinary team Complete accurate and timely documentation using WellSky What We're Looking For Active Virginia Physical Therapist License Required Graduate of an accredited Physical Therapy program Valid Driver's License and reliable transportation Strong clinical and communication skills Ability to work independently in patients' homes Home Health Experience Preferred, but Not Required WellSky Experience is a Plus Why Human Touch?
