Hospice RN Case Manager — Compassionate Home Care
St. Croix Hospice
St. Croix Hospice seeks a compassionate Registered Nurse Case Manager to deliver in-home, 1:1 holistic care. You will partner with an interdisciplinary team to assess needs, guide end-of-life conversations, and ensure comfort with professionalism and empathy. A nursing degree and RN licensure are required, with hospice/home health experience preferred.
