Pay Range: $42.85 - $68.45 Depending on Years of Experience

JOB PURPOSE: The Senior Manager of Care Coordination plays a critical role in advancing quality, outcomes-driven, and accessible patient care within a value-based care framework. This position is responsible for planning, directing, and aligning the efforts of a diverse care team—including social workers, medical assistants, registered nurses, and program assistants—to ensure coordinated, efficient, and patient-centered services. Working closely with the Senior Director of Care Coordination and Vice President of Population Health and Chief Quality Officer, the Senior Manager ensures that workforce productivity and care delivery models are strategically aligned to organizational goals. By fostering collaboration across disciplines, monitoring performance, and driving continuous improvement, this role supports the delivery of high-quality healthcare that emphasizes measurable outcomes, equity, and patient satisfaction.

Essential Job Functions:

· Develops and implements strategies for efficient, effective, and patient-centered care coordination services.

· Collaborates with all senior leadership to align care coordination efforts with organizational goals and value-based care initiatives.

· Designs and refines care delivery models that promote equity, measurable outcomes, patient satisfaction and value based care.

· Establishes strategic goals by gathering pertinent business, financial, service, and operations information; identifying and evaluating trends, choosing a course of action, and evaluating outcomes.

· Supervises and manages the care coordination team—including social workers, medical assistants, registered nurses, and program assistants—to ensure the delivery of high-quality care and services.

· Provides coaching, mentorship, and professional development opportunities to support team growth and performance.

· Fosters a collaborative, accountable, and excellence-driven team culture.

· Works in partnership with Information Technology (IT) to enhance and oversee value-based objectives, while advancing value-driven initiatives and productivity.

· Monitors and optimizes workforce productivity, ensuring appropriate staffing and resource allocation.

· Establishes and maintains workflows that support seamless care transitions and interdisciplinary collaboration to support positive outcomes.

· Ensures compliance with regulatory requirements and internal policies related to care coordination.

· Tracks and analyzes key performance indicators (KPIs) related to patient outcomes, satisfaction, and operational efficiency.

· Leads continuous improvement initiatives to enhance care quality and reduce unnecessary utilization.

· Leverages data analytics to identify trends, gaps, and opportunities for improvement.

· Collaborates efficiently with El Rio leadership, Health Center Managers, Human Resources, Finance, and other departments as needed to ensure optimal performance.

· Maintains professional relations by facilitating open channels of communication with other organizational leaders, departments, physicians, and ancillary personnel to promote value-based care throughout the organization.

· Serves as a liaison between care coordination teams, leadership, clinical departments, and external partners.

· Advocates for patient needs and ensure services are culturally competent and accessible.

· Supports integration of behavioral health, community resources and social drivers of health into care planning.

· Contributes to organizational effectiveness by identifying short-term and long-range issues that must be addressed.

· Provides information and commentary pertinent to deliberations, recommend options and courses of action, and implement directives

· Maintains a clean, safe, and hygienic work environment in compliance with all Policies and Procedures including but not limited to work areas, workstations, examination rooms, hand washing, infection prevention and control etc. for this position.

· Demonstrates an understanding of and proficiency with the application of all compliance and reporting requirements respective to Joint Commission Certification (JCC) standards.

Minimum Education and Experience:

· Bachelor’s degree in nursing from an accredited college or university.

· Seven (7) years’ experience as a Registered Nurse in an acute care, outpatient, or ambulatory setting

· Five (5) years of management experience in acute and/or out-patient case management;

· Five (5) years managing medical and nursing personnel

If applicable, equivalent combination of education and experience may be considered, and must be directly related to the functions and responsibilities of the job.

Required Licenses, Certifications, and Registrations:

· Current Registered Nurse license in good standing from Arizona State Board of Nursing.

· Current certification in Basic Life Support (BLS) for health care providers.

· Level I fingerprint clearance card: current valid and in good standing or have applied for it within seven working days after beginning employment.

· Employees in this position are required to have reliable transportation that can meet any operational reassignments of the organization during the workday. If an employee is driving during work hours, the employee is required to possess a valid driver’s license and must comply with Arizona vehicle insurance requirements.

Preferred Education, Experience, Skills, Abilities:

· Master’s Degree in Nursing from an accredited college or university.

· Ten (10) years’ experience as a Registered Nurse in an acute care, outpatient, or ambulatory setting.

· Seven (7) years’ management experience in acute and/or out-patient case management.

· Seven (7) of those years managing medical and nursing personnel.

· Bilingual (English/Spanish) with the ability to speak, read and write in both languages.
