Locations: Northern Nevada Including Reno, Sparks, Carson City, and Surrounding Areas

Make a difference where it matters most — at home. We’re looking for a dedicated Physical Therapist (PT) to join our compassionate home health team. Help patients regain mobility, independence, and quality of life — all from the comfort of their own homes.

Main Responsibilities:

Conduct patient assessments, create and update care plans, and establish clear, measurable goals.

Teach patients and their families exercises and self-care techniques that enhance mobility and promote safety.

Complete home safety evaluations and provide recommendations for modifications when necessary.

Work closely with physicians, nurses, and the interdisciplinary team to support high-quality patient outcomes.

Ensure all visit notes and documentation are completed accurately and on time.

Join case conferences and regularly update the care team on patient progress.

Help patients and their families understand care goals, expectations, and ongoing progress.

Qualifications:

Current Physical Therapist (PT) license in Nevada (required).

Minimum 1 year of home health physical therapy experience (preferred).

Strong clinical, communication, and problem-solving skills.

Ability to work independently and manage a flexible caseload.

Valid driver’s license and reliable transportation.
